……不同于后两种疾患的个案（精神病和边缘状态），自恋人格困扰的个案基本上已获致一个统整的自体（cohesive self），并且已建构完成统整的、理想化的古典客体（archaic object）。其次，不同于在精神病与边缘状态普遍见到的状况，古典自体或灌注（力比多）以自恋力比多的古典客体产生不可逆的解体（irreversible disintegration）的可能性，对这些个案并不构成严重威胁。

由于已获致这些统整而稳定的精神结构，这些个案能够建立特定的、稳定的自恋移情Narcissistic transference，这使得古典结构得以在治疗中重新激活reactivation，却不致有继续退行至解体fragmentation的危险，因此他们是可分析的。

……（精神病和边缘状态、自恋人格困扰）决定性的区别特征如下：（a）前者倾向于长期放弃统整的自恋结构，而以妄想取代之（以逃避无可忍受的解体状态及古典自恋客体的失落）；（b）后者呈现轻微而暂时的摆荡，通常朝向部分解体，最多只会出现少许稍纵即逝的代偿妄想。……后者的精神能够维系相对健康古典的夸大性质，而前者则是冰冷傲慢、达精神病程度的夸大妄想。……在自恋人格困扰个案所形成的移情中，会呈现一个被自恋灌注（力比多）的、全知全能、被赞叹和理想化、情绪支撑的双亲影像，这算是相对上健康的；至于精神病，则呈现一个威力无穷的自体加害者与操控者，即有影响力的机器、其全知全能已变成冷淡、不共情、非人类的邪魔。

最容易被确认出来的理想化移情的变异，在起源学上，和理想化双亲影像较后期的困扰有关，特别是就在正常理想化双亲影像被内射，及理想化力比多被将用于超我理想化的过程之前、之中，或紧接之后。

……重要起源学上的创伤根源于父母的精神病理，特别是根源于父母的自恋固着。父母的病理和自恋需求，决定性的助长了儿童持续过度和延长地交织于父母人格的自恋网中，直到父母突然的撤出，或是儿童突然绝望地体认他的情绪发展已经多么的混乱，他无法由一种长期自恋的关系获得全面的转变内化作用，他必须面对这项无法达成的任务，他之前曾经想从这种长期自恋的关系中脱身，但是没有成功。
